Zusammenfassung

Die Leberzhirrhose im Endstadium stellt die Hauptindikation für eine orthotope Lebertransplantation (OLT) dar. Als häufige Komplikation, die mit einer erhöhten Mortalität der Patienten mit Virushepatitis einhergeht, kommt es zu einer Reinfektion des Transplantats (Demetris et al. 1986; Portmann et al. 1986). Daher sind Patienten mit persistierender Hepatitis-B-Virus(HBV-)Replikation an einigen Kliniken von der OLT ausgeschlossen worden (van Thiel et al. 1984; Iwatsuki et a. 1988). Die Reinfektion des Transplantats mit Hepatitis-Delta-Virus (HDV) konnte man bei einem großen Anteil der transplantierten Patienten mit HDV-Begleitinfektion unabhängig vom Status der HBV-Replikation beobachten (Rizetto et al. 1987; Colledan et al. 1987; Colledan et al. 1989; Reyne et al. 1989; Agnes et al. 1989; Ferla et al. 1988; Zignego et al. 1990). Das histologische Spektrum der Transplantatschädigung nach Reinfektion mit Hepatitisviren reichte von der schwachen Mesenchymreaktion über die akute Hepatitis unterschiedlicher Ausprägung bis zur chronischen Hepatitis mit in einigen Fällen rascher Progression zur Zirrhose (Demetris et al. 1986; Lautz et al. 1989).
