Zusammenfassung

Die chronische Hepatitis B ist eine Lebererkrankung, die langfristig bei etwa 30 % der Patienten zu einer Leberzirrhose führt. Das Risiko der Entwicklung eines Leberzellkarzinoms ist bei Patienten mit chronischer Hepatitis B gegenüber der Allgemeinbevölkerung um mehr als das 100fache erhöht. Diese möglichen Komplikationen der chronischen Infektion mit dem Hepatitis-B-Virus (HBV) hat eine deutliche Einschränkung der Lebenserwartung zur Folge. Die Prognose hängt von der Serologie und der Aktivität der Hepatitis ab. Patienten mit positivem HBeAg und chronisch aktiver Hepatitis haben eine deutlich schlechtere Lebenserwartung als Patienten mit negativem HBeAg und chronisch persistierender Hepatitis. Auch bei nur geringer entzündlicher Aktivität besteht aber die Gefahr der Reaktivierung der Virusinfektion. Neben der Einschränkung der Lebenserwartung ist die chronische Hepatitis B häufig mit Beschwerden und Symptomen für den Patienten verbunden. Die eingeschränkte Leistungsfähigkeit und vermehrte Müdigkeit führen nicht selten zum Arbeitsausfall und sogar zur Berentung. Die Patienten mit aktiver HBV-Infektion stellen außerdem eine Infektionsquelle für andere Personen dar. Aus den genannten Gründen (Tabelle 1) wird seit vielen Jahren versucht, eine antivirale Therapie gegen die chronische Virusinfektion mit dem Hepatitis-B-Virus zu finden.
