Vorläufige Ergebnisse nach Implantation retroiridal irisfixierter Hinterkammerlinsen

Zusammenfassung

Bei insgesamt 27 Patienten haben wir eine retroiridal irisfixierte Hinterkammerlinse implantiert. Bei 12 dieser Patienten wurde im gleichen Eingriff eine perforierende Keratoplastik durchgeführt. Die Nachbeobachtungszeit der 8 Männer und 19 Frauen betrug zwischen 3 und 15 Monaten (m = 9,3 Monate). Wir verwendeten Hinterkammerlinsen mit 7-mm-Optik, modifizierten C-Schlaufen mit zwei Positionierungslöchern und fixierten sie mit zwei doppelt armierten (BV-4-Nadeln) 9-0-Prolenefäden durch die Positionierungslöcher. Das Durchstechen der Irisnaht erfolgt von dorsal und der Knoten wird ventral auf die Iris gelegt.

Der schnelle Anstieg der Sehschärfe der Patienten ohne Keratoplastik zeigt, daß der operative Eingriff von der Mehrzahl der Augen gut vertragen wird. Eine zystoide Makulopathie war bei 6 Patienten vorbestehend (Zustand nach ICCE mit Vorderkammerlinsenim- plantation, chronischem Reizzustand, sekundärem Druckanstieg und Hornhautendothel-dekompensation) und ist nicht der Operationsmethode anzulasten. An Komplikationen waren leichte Verziehung der Pupille durch die Irisnähte (3 Patienten), Iris capture (1 Patient), hintere Synechien (1 Patient), Ablatio chorioideae (1 Patient), Seclusio pupillae (1 Patient) zu beobachten.

In Anbetracht des Schweregrades der vorbestehenden Pathologie und in Anbetracht der möglichen optischen Alternativen (Starbrille, Kontaktlinse) stellt das Verfahren der Implantation retroiridal irisfixierter Hinterkammerlinsen eine Bereicherung des operativen Repertoires zur optischen Rehabilitation und Wiederherstellung der Lebensqualität betroffener Patienten dar. Da derzeit Langzeitbeobachtungen noch ausstehen, empfehlen wir eine strenge Indikationsstellung und eine sorgfältige Patientenauswahl.

Summary

We have implanted irissuture-fixated posterior Chamber lens in 27 patients. With 12 of these patients we simultaneously performed a corneal transplantation. The follow-up was between three and 15 months (m = 9.3 months). We used posterior Chamber lenses with a 7 mm optic zone, modified c-loops and with two holes. The lens is sutured to the iris with two double-armed 9-0 prolene sutures. The iris is penetrated from behind and the knot is positioned on the anterior surface of the iris.

The quick rise of the visual acuity of the 15 patients without corneal transplantation shows, that the Operation is well tolerated by the majority of the eyes. In 6 patients cystoid macular edema was preexisting (following ICCE with anterior Chamber lens implantation, secundary glaucoma and corneal endothelial decompensation) and is not caused by the Operation procedure. Complications were slight distorsion of the pupil by the iris sutures (3 patients), iris capture (1 patient), posterior synechiae (1 patient), ablatio chorioideae (1 patient), seclusio pupillae (1 patient).

Taking into account the marked preexisting pathology and the possible alternatives for optical correction (glasses, contact lenses), the Operation procedure of iris suturing of a posterior Chamber IOL is a enrichment of our operative methods and is usefull in the optical rehabilitation of afflicted patients. Because to date long term follow-ups are missing, we recommend to carfully select the patients for this procedure.
