Zusammenfassung

Es wird ein thermisch-kontraktives Verfahren zur Astigmatismus- und Hyperopiekorrektur vorgestellt, welches mit einem bei 1,54 μm emittierenden Erbium- Glas-Laser arbeitet. Die Lichtabsorption der Hornhaut ermöglicht bei dieser Wellenlänge eine Eindringtiefe von 1 mm. Durch periphere Hornhautkoagulation unter Freilassung einer optischen Zone von 6,6 mm konnte bei enukleierten Schafsaugen eine Brechkraftzunahme von 2–4 dpt erzielt werden, durch Koagulation nur in einem Meridian wurde ein Astigmatismus bis 4 dpt induziert. Die histologische licht- und elektronenmikroskopische Untersuchung der Präparate zeigte sehr gleichmäßige kegelförmige Koagulationszonen über etwa 80–90% der Hornhautdicke beim Schaf (ca. 1 mm). Unsere vorläufigen Ergebnisse belegen, daß die Hornhautstromakoagulation mit dem Erbium-Glas-Laser zur Astigmatismus- und Hyperopiekorrektur prinzipiell geeignet ist.

Summary

We present a method for correction of astigmatism and hyperopia using an erbium-glass laser emitting at 1.54 μm to produce localized effects of corneal thermal contraction. Light absorption of corneal tissue at this wavelength results in a maximum transmission dephth of 1 mm. By means of coagulation in the corneal periphery with an optical zone of 6.6 mm we could achieve an increase in optical power of 2–4 dpt in enucleated sheep eyes. Coagulation in one meridian only induced astigmatism up to 4 dpt. Histology showed cone-shaped coagulation zones of 80–90% of corneal thickness (1 mm) in sheep eyes. Our preliminary results indicate that coagulation of the corneal stroma with an erbium-glass laser could prove useful for correction of hyperopia and astigmatism.
