Zusammenfassung
Es wird über das relativ neue Verfahren der Endophako berichtet. Ursprünglich von Hara u. Hara [4] entwickelt, wurde es vor allem von Patel et al. [11] und Michelson [9] perfektioniert. Der Verfasser wendet das Verfahren seit einem halben Jahr als seine ausschließliche Methode der Kataraktchirurgie an. Das Verfahren besteht in der Emulsifikation der Linse im weitgehend geschlossenen Kapselsack, während die Vorderkammer durch Methylzellulose oder viskoelastisches Material versiegelt ist. Entscheidende Schritte der Operation sind die Kapseleröffnung durch eine Minirhexis von ca. 1 × 4 mm, die Hydrodissektion und Hydrodemarkation des Linsenkerns, die (einhändige) Phakoemulsifikation des so von der Kapsel gelösten Kerns und die Rindenaspiration durch die Minirhexis, die ausgiebige Reinigung und Polierung der Vorder- und Hinterkapsel, die Implantation einer geeigneten IOL durch die noch nicht erweiterte Kapselöffnung und schließlich die Erweiterung der Kapsulorhexis mit Schere und Rhexispinzette. Das Verfahren erweist sich als außerordentlich schonend für das Endothel. Während der Lernphase sind Kapselrupturen das größte Problem. Diese lassen sich aber gerade bei der Endophako relativ gut meistern. Bei entsprechender Erfahrung ist die Methode für praktisch alle Fälle der Kataraktchirurgie geeignet, auch für harte Linsen und enge Pupillen.
Summary
This is a report on the relatively new method of endo phako. Originally developed by Hara and Hara [4], the procedure was improved and perfected by Patel et al. [11] and Michelson [9]. The author has been using this method as his exclusive form of cataract surgery for over 6 months now. The method consists of the emulsification of the lens in the closed capsular bag, while the anterior Chamber is almost completely sealed by methyl cellulose or visco-elastic. Decisive steps of the procedure are opening of the anterior capsule by a minirhexis of appr. 1x4 mm, hydrodissection and hydrodemarcation of the nucleus, (one-handed) phako-emulsification of the nucleus and aspiration of the cortex through the intact minirhexis, diligent cleaning and polishing of the anterior and posterior capsules, in-the-bag implantation of a suitable IOL through the still un-enlarged rhexis and finally enlargement of the minirhexis with scissors and forceps. The method is extremely protective for the corneal endothelium. Düring the learning phase capsular ruptures may occur. But there are some advantages in handling such problems during an endo phako procedure. With appropriate experience, the method can be used in practically all cases of cataract surgery, even with hard nuclei and narrow pupils.
