Zusammenfassung
Bei Kleinschnitt-Kataraktchirurgie wird die Kunstlinse nach Phakoemulsifikation durch eine möglichst kleine Öffnung hindurch implantiert. Im Idealfall sollte man den Phakoemulsifikationsschnitt gar nicht mehr vergrößern müssen. Derzeit braucht man jedoch für die Implantation noch Schnittlängen zwischen 3,5 und 4,5 mm. 5 mm lange Schnitte werden meist auch noch der Kleinschnitt-Technik zugeordnet. Die Vorteile einer kleinen Schnittöffnung sind offensichtlich. Das geringe Gewebetrauma bewirkt eine kürzeren Heilungsverlauf und damit eine schnellere Rehabilitation des Patienten. Der operativ induzierte Astigmatismus ist sehr gering und bildet sich schnell wieder zurück. Durch spezielle Schnittführung und Nahttechnik (Horizontalnaht) läßt sich dieser Astigmatismus weiter reduzieren. Eine neuerdings angewendete nahtfreie Operationstechnik scheint in dieser Beziehung die besten Ergebnisse zu erbringen. Die derzeit verwendeten Linsen, die in einer Faltpinzette gefaltet oder in einer speziellen Spritze komprimiert durch die kleine Wundöffnung implantiert werden, sind aus weichen flexiblen Materialien wie p-Hema oder Silikon. Bei zeitgemäß optimaler Operationstechnik (sichere Kapselsackimplantation nach „continuous circular capsulorhexis“) werden sehr gute Erfolge erzielt.
Summary
Small-incision cataract surgery means IOL implantation through a wound opening which is not much longer than the incision for phacoemulsification. It would be the optimum to implant the IOL directly the phacoincision. But nowadays wound openings between 3.5 and 4.5 mm are required. Also 5 mm incisions sometimes are subsummized to small-incision cataract surgery. The advantages of this new technique are evident. Minimal tissue trauma results in an accelerated healing process and in early physical rehabilitation. The operative induced astigmatism is minimal and degrades postoperatively within a short time. With a special incision and wound closure technique (horizontal suture) it is possible to reduce astigmatism further more. The so called “no suture technique” seems to provide the best results. At the present the commonly used foldable IOLs for small incision technique surgery are p-Hema lenses and silicone lenses. The implantation is performed with a special folding forceps or with a syringe type instrument. In combination with a proper surgical technique (continuous circular capsulorhexis, capsular bag implantation) the results are excellent.
