Zusammenfassung
Die Implantation bi- oder multifokaler IOLs gewinnt seit einigen Jahren zunehmend an Interesse. Ein neuartiges Design aus Silikon ermöglicht die Implantation nach Phakoemulsifikation durch eine 4-mm-Inzision. Von August bis Dezember 1990 implantierten wir im Rahmen einer prospektiven Studie 10 varifokale Silikon-IOLs (Typ VF 100, Fa. Wright Medical, Inc.; 6 mm asphärische Optik aus Silikon, 2 Prolene-Schlaufen). 4–8 Wochen post-operativ bestimmten wir die Sehschärfe bei Defokussierung sowie die Kontrastsehschärfe (KS) mittels der Regan-Charts (96%, 50%, 25%, 11%).
Der durchschnittliche Astigmatismus betrug 0,35 D, der Fernvisus mit Korrektur 0,92, der Nahvisus mit Fernkorrektur Nieden 5, mit Nahaddition von 2,25 D Nieden 2. Die Defokussierkurve zeigte ein Maximum bei Fernkorrektur und fiel bei Zugabe von Minusgläsern kontinuierlich ab, ein zweiter Gipfel im Nahbereich fehlte. Ein Visus von 0,5 oder besser wurde über einen Bereich von 3 D (+1,0 D bis -2,0 D) erreicht. Die KS war bei niedrigem Kontrast (11%) leicht reduziert.
Unsere Ergebnisse zeigen einen guten Fernvisus mit dieser multifokalen IOL bei durch den kleinen Schnitt bedingtem geringem Astigmatismus. Der Nahteil ist zu schwach dimensioniert, bei Defokussierung fehlt der Gipfel im Nahbereich. Es zeigte sich jedoch eine brauchbare Sehschärfe in mittleren Distanzen.
Summary
During the last years a number of bifocal IOL has been introduced. A new varifocal design made of silicone can be folded and implanted through a 4 mm incision. In a prospective study we implanted 10 varifocal IOLs (VF 100, Wright Medical Inc., 6 mm silicone optic, 2 prolene loops) between Aug. and Dec. 1990. 4–8 weeks post-op we determined the defocus curve, contrast acuity (CA) using the Regan Charts (96%, 50%, 25%, 11%), astigmatism and visual acuities.
Best corrected distande acuity was 20/22, astigmatism 0.35 D, near acuity with distance correction 20/50, with an average near-add of 2.25 D acuity was 20/29. The defocus curve peaked at distance correction and decreased continuously with defocus. There was no second peak at near. Acuity was 20/40 or better in a ränge of 3 D (+1.0 D to -2.0 D). CA was slightly reduced at low contrast levels (11%).
Our results indicate an insufficient near-add of the VF 100 IOL. However, useful vision is provided in intermediate distances due to an increased depths of focus.
