Zusammenfassung
Die Implantation bifokaler IOLs gewinnt zunehmendes Interesse. Die TRUE VISTA IOL, eine refraktive Bifokal-IOL mit einem zentralen und peripherem Fernteil sowie einem ringförmigen Nahteil, wurde kürzlich vorgestellt.
Im Rahmen einer Europäischen Multizentrischen Studie wurden von März bis September 1990 165 Linsen (165 Patienten) von 13 Operateuren implantiert. Bis Oktober konnten 50 Patienten ohne sonstige Augenerkrankungen (best cases) 4–6 Monate postoperativ nachkontrolliert werden. Die korrigierte Fernsehschärfe war in 98% 0,5 oder besser, in 90% 0,63 oder besser und in 48% 1,0 oder besser. Die korrigierte Nahsehschärfe war in 100% 0,63 oder besser, in 98% 0,8 oder besser und in 68% 1,0 oder besser. Die unkorrigierte Nahsehschärfe war in 100% 0,63 oder besser und in 91% 0,8 oder besser.
Hinsichtlich des korrigierten Nahvisus fand sich eine Korrelation zum postoperativen Astigmatismus. Mit steigendem Astigmatismus fiel die Sehschärfe trotz optimaler Korrekturab. Ebenfalls deutlich korreliert waren Patientenalter und korrigierter Visus, Visuswerte unter 0,63 (Ferne) bzw. unter 0,8 (Nähe) fanden sich nur bei über 70jährigen Patienten.
Unsere Ergebnisse zeigen eine gute Wirksamkeit der TRUE VISTA Bifokal-IOL. Zusätzlich wird deutlich, daß ein bestehender Astigmatismus sowie das Patientenalter die Funktion bifokaler IOLs zu beeinflussen scheint.
Summary
During the last years a number of bifocal IOLs was developed. The TRUE VISTA IOL is a three-zone refractive bifocal IOL with a central and peripheral distance zone and a pericentral near-annulus. In a prospective European Multicenter Study 165 TRUE VISTA IOLs were implanted between March and Sept. 1990. 50 best case patients were available for 4–6 months follow-up until now.
Best corrected distance acuity was 20/40 or better in 98%, 20/30 or better in 90% and 20/20 or better in 48%. Best corrected near acuity was 20/30 or better in 100%, 20/25 or better in 98% and 20/20 or better in 68%. Uncorrected near acuity was 20/30 or better in 100% and 20/25 or better in 91%.
Near acuity correlated with postoperative astigmatism. Acuity decreased with increasing astigmatism. Near and distance acuity also correlated with age. Acuities below 20/30 (distance) and 20/25 (near) were found in patients above 70 years of age only.
Our results suggest a good visual Performance with the TRUE VISTA Bifokal IOL. However, postoperative astigmatism and patient age seems to influence the Performance of bifocal IOLs.
