Zusammenfassung

Kasuistik einer 73jährigen Patientin, bei der eine komplikationslose Phakoemulsifikation durchgeführt wurde. Der postoperative Verlauf während der ersten 3 Monate war unauffällig, der Visus betrug ohne Korrektur 0,8. In den darauffolgenden 2 Jahren kam es zu einer zunehmenden Myopisierung bis auf −2,5 sph bei äußerlich reizfreiem Auge. Bei der vom Optiker veranlaßten augenärztlichen Wiedervorstellung zeigte sich, daß bei reizfreiem, normotonen Auge, die Hinterkammerlinse fast vollständig durch die Iris hindurch in die Vorderkammer erodiert war. Anhand der Haptikenden wurde rückgeschlossen, daß der Pseudophakos invers mit der Bügelabwinkelung nach hinten implantiert worden war.

Summary

Case report of a 73 year old female patient who underwent uncomplicated phacoemulsification with implantation of a posterior Chamber lens. Postoperative follow-up during the first 3 months was uneventful; vision increased to 0.8 (20/25) without glasses. During the following two years the patient’s optician noted a progressive myopization to −2.5 sph in an otherwise asymptomatic eye and initiated a medical check-up. On presentation at our clinic we saw that the posterior Chamber lens had eroded almost completely through the iris into the anterior Chamber. There were no signs of intraocular inflammation and the eye showed normal pressure. According to the shape and location of the haptics it was found that the lens had been implanted inversely, with the haptics angulated backwards, pressing the lens against the iris. Due to the uninflamed status of the eye with good vision and the high rate of severe complications to be expected on explantation of the lens, no further surgery was performed.
