Pars-plana-Vitrektomie bei in den Glaskörper luxierten Hinterkammerlinsen

Zusammenfassung

In 7 Fällen mit Luxation von Hinterkammerlinsen in den Glaskörperraum wurde im Rahmen der chirurgischen Versorgung eine Pars-plana-Vitrektomie durchgeführt. Die mittlere Beobachtungszeit beträgt 15 Monate nach dem Eingriff. Ursache für die Luxation der Hinterkammerlinsen war bei 2 Patienten eine YAG-Laserkapsulotomie, bei 2 Patienten eine spontane Luxation, bei 3 Patienten war die Linse 1–3 Tage nach Kataraktoperation in den Glaskörper luxiert. Bei 2 Fällen konnte die abgetauchte Linse in den Sulkus reponiert werden. In 5 Fällen wurde die Linse entfernt und die neue Linse durch Sulkusnähte fixiert. Bei 2 Patienten kam es im Beobachtungszeitraum zu einer progredienten epiretinalen Gliose, davon bei 1 Patient zu einem Makulaforamen bei vorliegender seniler Makuladegeneration. Eine umfassende Vitrektomie ist unserer Ansicht nach von entscheidender Bedeutung, um späteren vitreoretinalen Komplikationen vorzubeugen.

Summary

Seven patients with posteriorly dislocated intraocular lenses had repositioning or removel of the IOL using pars plana instrumentation. The mean follow up time postoperatively was 15 months. The cause for dislocation of the IOLs was in 2 patients neodymium YAG laser capsulotomy, in 2 patients spontaneous luxation and in 3 patients luxation of the IOL into the vitreous 1 to 3 days postoperatively. In two cases the posteriorly dislocated IOL’s could be repositioned into the ciliary sulcus. In 5 cases the IOL’s were removed and replaced by new posteriorChamber lenses fixated with sulcus sutures. In 2 patients a progressiv epiretinal gliosis could be observed, of which in 1 patient the final functionai result was impaired by a macular hole associated with macular degeneration. It appears that a careful pars plana vitrectomy reduces the danger of late retinal complications.
