Zusammenfassung
Die meisten Untersuchungen zur Pathogenese der Osteoarthrose (OA) haben die Analyse des hyalinen Knorpels zum Inhalt, welcher jedoch nur einen Teil des Gelenksystems darstellt. Eine vermehrte Bedeutung wird inzwischen den angrenzenden Strukturen beigemessen. So erfüllt die Tidemark als Bindeglied zwischen hyalinem und verkalktem Knorpel verschiedene physiologische Funktionen (Mineralisationsbarriere, Verankerung des weichen hyalinen Knorpels mit der rigiden Kalknorpelschicht) und ist unmittelbar mit dem Prozeß der Knorpeldestruktion bei OA verknüpft. In der Literatur werden Tidemarkschädigungen sowie Veränderungen des subchondralen Knochens als Mechanismen diskutiert, welche sekundär die Knorpeldegeneration hervorrufen können [1–5].
