Stellenwert der Schleifendiuretika in der Hypertoniebehandlung

Zusammenfassung

Diuretika werden seit ca. 30 Jahren erfolgreich in der Hypertoniebehandlung eingesetzt. Zahlreiche Interventionsstudien zur antihypertensive Therapie, hauptsächlich durchgeführt mit Diuretika und β-Blockern, zeigten eine klare Reduktion des Apoplexrisikos sowie der Herzinsuffizienz, konnten jedoch keine Verbesserung der koronaren Herzkrankheit (KHK) zeigen (McMahon 1990; Collins et al. 1990; Amery et al. 1990). Andererseits fehlen prospektive Studien zu neueren Antihypertonika wie ACE-Hemmer und Kalziumantagonisten in Hinsicht auf Beeinflussung von Herz-Kreislauf-Morbidität bzw. Mortalität von Hypertonikern. Aufgrund ihrer vergleichbaren Wirksamkeit und möglichen metabolischen Vorteile gegenüber Thiaziden und β-Blockern werden sie jedoch schon heute in der ersten Stufe zusammen mit β-Blockern und Diuretika empfohlen. Es gibt allerdings Stimmen, die weiterhin aufgrund fehlender Ergebnisse von Interventionsstudien strikt Diuretika und β-Blocker als First-line-Therapie empfehlen, die anderen Klassen nur bei Kontraindikationen oder Ineffektivität der beiden genannten (Swales 1990).
