Zusammenfassung
Nach Resektionen am oberen Gastrointestinaltrakt treten bei einem Teil der Patienten postprandial Kreislaufsymptome auf, das frühe Dumping-Syndrom, die mit einem akuten Abfall des Plasmavolumens einhergehen [1]. Die rasche Entleerung des Restmagens bzw. des Magenersatzes spielt die Schlüsselrolle bei diesem Phänomen. Bei einem Teil dieser Patienten kommt es zu reaktiven Hypoglykämien, deren Pathogenese nicht restlos geklärt ist Die Insulinfreisetzung stimulierende Faktoren aus dem Darm werden unter anderem dafür verantwortlich gemacht, und kürzlich wurde das Glucagon-like-peptide-1 (GLP-1), ein Spaltprodukt des Proglucagon, als starkes insulinotropes Hormon aus dem distalen Dünndarm identifiziert [2]. Es war Ziel dieser Studie, die Zusammenhänge zwischen der Entleerung des Magenersatzes, postprandialer Symptomatik, und GLP-1-Freisetzung zu beleuchten.
