Abstract
Favorable results in the recent trials of streptokinase and urokinase [6, 7, 9, 12, 15, 16] and the development of recombinant tissue-type plasminogen activator (rt-PA) [2, 3, 5, 8, 13] have renewed interest in thrombolytic therapy in ischemic stroke. I describe here my latest findings on fibrinolytic recanalization therapy in acute stroke. The first section describes local intraarterial urokinase plasminogen activator (u-PA) infusion with acute stroke, and the second section outlines the progress which has been made in our ongoing systemic recombinant tissue-type plasminogen activator (rt-PA) trial in acute carotid territory stroke. Basic issues we must resolve with our clinical trials are first whether early recanalization is followed by clinical improvement and second whether fibrinolytic agents can adequately and safely recanalize occluded arteries.
