Abstract
Leukoaraiosis (LA) [5] is a purely descriptive term which applies to computer tomography (CT) images with reduced density in the white matter, around the lateral ventricles, and in the centra semiovalia, and is frequently observed in elderly subjects. The early studies reporting this CT finding, with its clinical and pathological correlates, considered LA as the in vivo documentation of Binswanger’s encephalopathy (BE) [14], characterized by dementia, strokes, and subacute accumulation of focal deficits [3]. More recent observations, however, [8] indicate that LA is not invariably associated with stroke or dementia. It is now certain that LA may be encountered in patients with a broad spectrum of clinical conditions, from healthy aged subjects to severely demented patients. Since the pathophysiology of LA remains to be elucidated, these discrepancies are far from being explained. This paper examines the relation of stroke to LA, also taking into account dementia in stroke patients.
