Zusammenfassung
Die Immunszintigraphie gastrointestinaler Tumoren besitzt bis heute ausschließlich Bedeutung für das kolorektale Karzinom, da für die übrigen Tumorgruppen keine entsprechenden Antikörper allgemein verfügbar sind, die eine gute Darstellbarkeit ermöglichen. Deshalb soll sich dieser Beitrag auf kolorektale Karzinome beschränken. Die Prognose von Patienten mit kolorektalen Karzinomen hat sich in den letzten Jahrzehnten trotz großer Bemühungen auf dem Gebiet der Diagnostik und Therapie kaum gebessert [19]. Das kolorektale Karzinom steht in Deutschland für beide Geschlechter an der zweiten Stelle in der Mortalitätsstatistik der bösartigen Neubildungen [28]. Die histologische Einteilung kolorektaler Karzinome geht weltweit nach der TNMKlasssifikation bzw. dem Dukes-Schema. Die 5-Jahres-Überlebensrate hängt von dem Stadium der Erkrankung bei Diagnosestellung ab. So beträgt sie im Stadium Dukes A (Tumorinfiltration der Submukosa) 85%, bei Dukes D (Fernmetastasen) nur 5% [12]. Eine wesentliche Verbesserung der Prognose scheint hierbei nur durch eine frühzeitige Diagnose und Rezidivprophylaxe mit Erkennung von Mikrometastasen möglich. Das carzinoembryonale Antigen (CEA), bereits 1965 durch Gold und Freedman als tumor-assoziiertes Antigen des Gastrointestinaltrakts beschrieben [7], wird postoperativ zur Rezidiverkennung im Serum bestimmt. Der CEA-Spiegel ist dabei in etwa 75% entweder gleichzeitig oder in einzelnen Fällen bereits vor dem Auftreten eines nachweisbaren Tumorrezidivs erhöht [20].
