Abdominoperineale Rektumexstirpation versus Rektumresektion: Geringere Lokalrezidivquote des radikaleren Verfahrens

Zusammenfassung

Im Verlauf der letzten Jahre zeichnete sich in der Chirurgie des Rektumkarzinoms des mittleren und distalen Rektumdrittels eine deutliche Favorisierung der Rektumkontinenzresektion (im Folgenden RR abgekürzt) gegenüber der abdominoperinealen Rektumexstirpation (im Folgenden APR abgekürzt) ab, wobei RR auch bei T3-Tumorstadien, bei Nachweis von Lymphknotenfiliae und auch bei lateraler Gewebeinfiltration durchgeführt wurden [20, 23, 26, 36]. Mit Änderung der Wahl des Operationsverfahrens und dem Zwang zum Anlegen tiefer Rektumanastomosen trat eine Diskussion über die Länge des aboralen Sicherheitsabstands zum Tumor auf, die klassische „5-cm-Regel“ mußte teilweise verlassen werden [11, 17, 23, 35, 38]. Der teilweise höheren Lokalrezidivquote nach RR folgten in der Literatur Empfehlungen zur Steigerung der Radikalität durch weite laterale Resektion und Mitresektion des Mesorektums, neuerlich wird auch die Lymphonodektomie im MesentericaInferior-Gebiet als Mittel zur Verhinderung lokoregionärer Rezidivtumore diskutiert und bei Rektumkarzinomen im unteren Drittel die knappe Kontinenzresektion erprobt [1, 2, 7, 12, 23, 27, 29, 33, 38]. Neben dem sphinktererhaltenden Effekt der RR sollten bei der Wahl des Operationsverfahrens auch die Folgen des Lokalrezidivs für die eventuelle Rezidivoperation und die Lebensqualität des Patienten in Betracht gezogen werden [22]. Unter diesem Aspekt erfolgte die Auswertung des Patientenguts.
