Zusammenfassung
Aufgrund der hohen Komplikationsdichte der radikalen Prostatektomie, die in den 70er Jahren berichtet wurde [1, 12], und der Auffassung vom Prostatakarzinom als „benign killer“, wurde die Therapie im 8. Lebensjahrzehnt nicht mehr in Betracht gezogen [8, 14]. Tatsächlich scheint sich aus klinischer Sicht die Dignität dieses Tumors eher altersneutral zu verhalten [21]. Middleton [19] hebt die gestiegene Lebenserwartung anhand der Berechnungen dreier großer amerikanischer Versicherungen -11,2 bis 15,5 Jahre für einen gesunden 70jährigen - hervor. In der BRD hatte 1987 ein 70jähriger Mann eine 78 %ige, und ein 75jähriger eine 65%ige Lebenserwartung von 5 Jahren, erst mit dem 80. Lebensjahr fällt die 5-Jahres-Überlebenswahrscheinlichkeit auf 50% (Abb. 1), so daß zwischen dem 75. und 80. Lebensjahr die kalendarische Altersbegrenzung für ausgedehnte operative Maßnahmen mit kurativer Zielsetzung anzunehmen sein dürfte. Jenseits des statistisch auszuschließenden Nutzens der RRP beim Hochbetagten wird dieser nicht nur mit den eingriffstypischen Nachwirkungen belastet (vide infra), sondern es sind von der betreuenden Klinik auch ganz besondere Aufwendungen zu treffen, um die physiologische Abnahme der Körperfunktionen im Alter abzufangen (Abb. 2). Tatsächlich scheint dies bei sorgfältiger Präselektion der betroffenen Patienten bemerkenswert gut zu gelingen, wie die Ergebnisse einer anderen beckenchirurgischen Maßnahme, der Zystektomie, und auch der Koronarchirurgie beim 80jährigen belegen [18, 30].
