Follow-up After Transurethral Resection for Stage A Prostatic Cancer

Abstract

The stage A2 carcinoma of the prostate is most commonly discovered incidentally on histological analysis of prostatic tissue removed during transurethral resection (TURP) performed for infravesical obstruction. On this occasion the far-reaching decision must be made whether a symptom-free patient with a tumor of perhaps only minor malignant potency should be subjected to curative, but invasive therapy, or whether to dispense with treatment completely and confine the activities to regular follow-up examinations. No compromise is possible between these two modalities. The final decision must be based on the following considerations. It must be possible to assess the statistical risk of dying from the cancer with a high degree of probability for the overall patient population as well as for the individual patient. The risk of dying from the tumor must be weighed against the risk of therapy including mortality but also the considerable morbidity involved in treatment. Another prerequisite for being able to take the responsibility for a wait-and-see strategy is the availability of diagnostic means for the detection of progression in a curable stage. This very essential problem is discussed here.
