Früherkennung der idiopathischen Femurkopfnekrose — Voraussetzung für die lokale Nekroseausräumung

Zusammenfassung

Ist die Femurkopfnekrose bei konventioneller Röntgentechnik manifest, so haben sich bereits morphologische Sekundärreaktionen um den nekrotischen Herd gebildet In fortgeschrittenen Stadien ist außerdem die Femurkopfkontur durch den Einbruch der Nekrose zerstört. Um das Fortschreiten der nekrotischen Destruktion zu verhindern und bei noch begrenzter Nekrose den Femurkopf zu erhalten, wird üblicherweise eine intertrochantere Osteotomie zur Verbesserung der Belastungsverhältnisse und Erhalt eines guten Containments durchgeführt. Verlaufskontrollen zeigen eine deutlich erhöhte Komplikationsrate mit verzögerter knöcherner Konsolidierung und Pseudarthrosenentwicklung. Im weiteren Verlauf und bei ausgeprägter Nekrotisierung bleibt schließlich trotz des relativ jungen Alters der Betroffenen oft nur eine endoprothetische Versorgung.
