Hormonelle Folgetherapien nach Goserelin (Zoladex®) beim metastasierten Mammakarzinom prämenopausaler Patientinnen

Zusammenfassung

Der GnRH-Agonist Zoladex® (Z) (3,6 mg Goserelin; slow releasing depot-Präparat, s.c. alle 4 Wochen appliziert) hat sich als effektive Alternative zu den konventionellen hormonablativen Verfahren (Ovarektomie, Radiokastration) bei prämenopausalen Patientinnen mit metastasiertem Mammakarzinom erwiesen. Voraussetzung ist dabei eine nicht akut lebensbedrohliche, nicht foudroyant verlaufende Metastasierung. Die Vorteile Hegen in der temporären und damit reversiblen Wirksamkeit. Ein außerdem zu postulierender direkter Wirkmechanismus über GnRH-Rezeptoren der Tumorzellen kann eine bessere Effektivität im Vergleich zur Ovarektomie und eventuell die Wirksamkeit in der Postmenopause erklären. Die bisher beschriebenen Ansprechraten (RR: komplette und partielle Remission: CR + PR) liegen bei 45%1.
