Phakoemulsifikation und Implantation bei voroperierten Augen

Zusammenfassung

In der Kataraktchirurgie stellt der operative Zugang und die Schnittführung bei Augen mit funktionierendem Sickerkissen nach vorangegangener fistulierender Glaukomoperation ein gewisses Problem dar. Es wird die Technik der Phakoemulsifikation und der anschließenden Implantation einer Intraokularlinse in den Kapselsack demonstriert. Nach Anlegen eines peripheren cornealen Schnittes unter Schonung des Sickerkissens erfolgt nach Lösen von eventuell vorhandenen hinteren Synechien die Kapsulorhexis. Die Phakoemulsifikation und der anschließende Absaugevorgang werden unter Zuhilfenahme des push-pull-Hakens vorgenommen, so daß eine Iridotomie und eine Irisnaht nicht erforderlich sind. nach Erweitern des Schnittes erfolgt die Implantation der Intraokularlinse in den Kapselsack. Vor- und Nachteile des Verfahrens sowie alternative Möglichkeiten werden dargestellt und diskutiert.

Summary

The surgical approach and the location of incisions in eyes with functioning filtering bleb after prior fistulating glaucoma operation constitutes a certain problem in cataract surgery. The technique of phacoemulsification and subsequent implantation of an intraocular lens into the capsular bag is demonstrated. After making a peripheral corneal incision avoiding damage to the filtering area, capsulorhexis is carried out after detaching any posterior synechias which may be present. The phacoemulsification and the subsequent aspiration procedure are carried out using the push-pull-hook, so that an iridotomy and an iris suture are not necessary. After extending the incision, the intraocular lens is implanted into the capsular bag. Advantages and disadvantages of the procedure as well as alternative methods are described and discussed.
