Die intrakapsuläre Blutung — Eine seltene Komplikation nach Kapsulorhexis und Kapselsackfixation einer plankonvexen Kunstlinse

Zusammenfassung

Kasuistische Mitteilung über vier Patienten, bei denen es nach komplikationsloser Kapsulorhexis und extrakapsulärer Kataraktextraktion mit Implantation einer Kunstlinse in den Kapselsack in den ersten postoperativen Tagen zu einem Hyphäma kam. Nach dessen Resorption zeigte sich eine ausgedehnte Blutansammlung zwischen der Rückfläche des Implantates und der hinteren Linsenkapsel. Pathogenetisch wird ein Zusammenhang zwischen großem Vorderkapselrest, endokapsulärer Fixation einer plankonvexen Kunstlinse und hierdurch möglicher Kapillarattraktion des Blutes in den Kapselsack postuliert. Nach Auflösung der Sanguination bestand bei allen vier Patienten eine ausgeprägte sekundäre Kapselfibrose, die eine YAG-Laserkapsulotomie notwendig machte.

Summary

Case report on four patients in whom a hyphema developed in the early postoperative period after an uneventful capsulorhexis and extracapsular cataract extraction with endocapsular IOL fixation. After resorption of the hyphema, a collection of blood between the posterior surface of the IOL and the posterior lens capsule was observed. In the pathogenesis of this complication, a connection between the large portion of the anterior capsule, the endocapsular fixation of a plano-convexe IOL and thereby a capillary attraction of blood into the capsular bag is postulated. In all patients an extensive secondary capsular fibrosis was present after the resorption of the blood, which had to be treated by YAG-Laser.
