Zusammenfassung

Eine postoperative Bulbushypotonie nach extrakapsulärer Cataraetex-traktion und gleichzeitiger HKL-Implantation ist ohne Zyclodialyse ein seltenes Ereignis. In einem Krankengut von 2700 konsekutiven E.C.C.E. plus HKL wird dies nur 4mal beobachtet.

Von 2 Patienten werden die Verläufe vorgestellt:

  1. 1.

    subjektive Symptome: metallische Lichtblitze

  2. 2.

    objektive Symptome: Visusabfall auf 0,4 bzw. 0,07

  3. 3.

    i.o.p. < als 6 mm Hg

  4. 4.

    Augenhintergrund: Aderhautabhebung, echographisch dokumentiert

  5. 5.

    Fleckblutungen sowie peripapilläres Oedem

Therapie: lokale differenzierte Steroidgabe, allgemein Antiphlogistika (Diclofenac) Restitutio ad integrum

Summary

Postoperative hypotony following e.c.c.e with posterior chamber lens implantation is a rare finding. Among 2700 consecutive e.c.c.e and p.c.l., this syndrome was observed only in 4 cases (0.15%). A cyclodialysis was not observed.

A clinically controlled follow up of the course of the disease was possible in two patients. Patients complaints:

  • — perception of light-flashes

  • — blurred vision

  • Objective findings:

  • — visual acuity decrease

  • — i.o.p. <6 mm Hg

  • — chorioidal detachment (echographically documented)

  • — retinal hemorrhage

  • — peripapillary edema

Therapy: topical steroids and systemic nonsteroidal (Diclofenac) antiphlogistic agents led to a full recovery with rise of visual acuity and i.o.p. into an optimal range
