Zusammenfassung
Um die effektivste präoperative Tropfenapplikation zu finden, wurde bei 31 Kataraktpatienten die Kammerwasserkonzentration von Diclofenac, einem Prostaglan-dinsynthetasehemmer, bestimmt. Das Patientenkollektiv wurde in fünf Gruppen von 5 bis 6 Personen unterteilt. Die Tropfengabe erfolgte in Gruppe I fünfmal in stündlichem Abstand am Vorabend der Operation, Gruppe II und III am Vorabend fünf- bzw. dreimal und präoperativ dreimal im halb- bis stündlichen Intervall, Gruppe IV und V nur am Operationstag drei- bzw. fünfmal. Die höchste Mittelwertkonzentration von 165,0 ng/g ergab sich für die Gruppe II. Den zweithöchsten Kammerwasserspiegel wies die Gruppe V mit 131,7 ng/g auf. Zur Erzielung hoher Kammerwasserkonzentrationen und Hemmung der intraokularen Prostaglandinsynthese sollten 0,1% Diclofenac-AT präoperativ fünfmal über drei bis fünf Stunden gegeben werden. In der postoperativen Behandlung müssen wegen der Halbwertzeit von vier bis sechs Stunden die Diclofenactropfen fünfmal pro Tag appliziert werden, um gleichbleibend hohe Kammerwasserspiegel zu erzielen.
Summary
The aqueous humor concentration was determined to evaluate the most effective preoperative topical treatment for 0,1% Diclofenac-Na, an inhibitor of prostaglandin synthesis. Thirty-one patients with a cataract were divided in five Groups of 5 to 6. Group I received five doses of 0,1 % Diclofenac at one hour intervals the evening before the operation. Group II and Group III were treated with 5 or 3 doses the evening before and preoperatively the next morning. In Group IV and Group V 3 or 5 drops were applied in half to one hour intervals presurgically. The highest aqueous humor concentration of Diclofenac was found in Group II (x = 165,0 ng/g). Five doses given preoperatively (Group V) produced the second highest concentration, x = 131,7 ng/g. To inhibit intraocular prostaglandin synthesis 0,1% Diclofenac eyedrops should be administered five times over three to five hours preoperatively. Continuously high aqueous humor concentrations of Diclofenac are achieved with five drops per day.
