Zusammenfassung
Fett erfüllt in der Ernährung eine unspezifische Aufgabe als Energieträger und Lieferant von Kohlenstoffatomen und eine spezifische als Träger fettlöslicher Wirkstoffe. Da jahrelang sehr intensiv und nahezu ausschließlich darüber diskutiert wurde, ob, wann und in welcher Dosierung Fett im Rahmen eines parenteralen Ernährungsregimes eingesetzt werden sollte, hat man ohne Zweifel der nichtenergetischen Bedeutung der Fette in der klinischen Routine zu wenig Beachtung geschenkt. Die einer Arbeit von Spielmann et al. (1988) entnommene Tabelle 1 zeigt, daß mittelkettige, langkettige, gesättigte sowie ein- oder mehrfach ungesättigte Fettsäuren unterschiedliche Bedeutung im Energiestoffwechsel als Strukturbausteine oder bei der Erfüllung spezieller Funktionen haben. Es Sci nur daran erinnert, daß Lipide Bestandteil der Zellmembran, der Mitochondrien, der Lysosomen und des endoplasmatischen Retikulums sind und daß ihnen eine besondere Bedeutung zukommt bei der Regulation des Stoffwechsels sowie bei der Differenzierung und des Wachstums, da die Steroidhormone der Nebennierenrinde und der Gonaden ebenso wie die Prostaglandine Lipide sind. Parenteral zugeführt besitzt Fett anderen Kalorienträgern gegenüber u.a. den Vorteil, daß es osmotisch nicht wirksam ist und somit über periphere Venen verabfolgt werden kann und daß selbst bei hoher Dosierung renale Verluste nicht auftreten.
