Zusammenfassung

Im wesentlichen einheitlich wird das initiale therapeutische Vorgehen bei der Behandlung des Grand-mal-Status mit Benzodiazepinen und/oder Phenytoin von verschiedenen Autoren angegeben, obwohl die Initialbehandlung mit Phenobarbital durchaus nicht weniger erfolgreich ist (Shaner et al. 1988). Nach Versagen von Benzodiazepinen und Phenytoin ist jedoch die Reihenfolge weiterer Maßnahmen unterschiedlich. Neben Chlomethiazol, das bei einem alkoholisch bedingten Status zum Einsatz kommt, stehen Barbiturate, Lidocain, Thiopental, seltener Isoflurane, Althesin oder Paraldehyd (in Deutschland nicht verfügbar) zur Auswahl (Uthman u. Wilder 1989; Treiman 1989; Browne 1979; Bleck 1983; Ramsay 1989).
