Zusammenfassung
Der Nachweis morphologisch faßbarer Gewebsveränderungen als Ursache oder Folge des Status epilepticus war von jeher von großem Interesse Ammonshornsklerosen wurden bereits von Pfleger (1880) und Spielmeyer (1927) als Folge epileptischer Anfälle angesehen. Scholz (1951) gelang der Nachweis, daß sich nach einem oder mehreren aufeinanderfolgenden Grand mal elektive Parenchymnekrosen als charakteristischer Sauerstoffmangeleffekt einstellen können. Dabei gehen nur die sauerstoffempfindichen Ganglienzellen einzeln oder in ganzen Populationen zugrunde, während das gliöse und mesodermale Interstitium intakt bleibt. Eine Schrumpfung der befallenen Territorien — abhängig vom Umfang der Ganglienzellausfälle — ist die Folge: narbige Oberflächeneinziehungen (granuläre Atrophie) in der Großhirnrinde bei gefäßabhängigen, massierten Ausfallsherden; Schrumpfung ganzer Windungen (Ulegyrien) bei gleichmäßigerem Ausfall oder Atrophie eines umschriebenen Hirnareals, z. B. dem Ammonshorn. Pfeiffer (1963) überprüfte diese Befunde und wies eine enge Korrelation zwischen der Anfallsfrequenz und dem Ausmaß der krampfabhängigen Gewebsschäden nach. Er erhob pathologische Befunde in 173 von 285 Fällen.
