Zusammenfassung
Rezidive von Zervixkarzinomen nach Operation und adjuvanter Strahlentherapie oder nach alleiniger primärer Strahlenbehandlung haben eine extrem schlechte Prognose und stellen eine Herausforderung für den gynäkologischen Onkologen dar. Bei ca. 35% der Patientinnen mit invasivem Zervixkarzinom zeigt sich im weiteren Krankheitsverlauf ein solches Rezidiv. Die Lokalisation dieser Rezidive betreffen in etwa einem Drittel der Fälle ausschließlich den Scheidenstumpf (27%) und den unteren Teil der Vagina (6%), über 40% der Rezidive sind überwiegend im Parametrium und an der Beckenwand lokalisiert. Nur ca. 20% der Patientinnen weisen Fernmetastasen auf [10, 27, 32, 33]. Treten solche Rezidive nach ausschließlich operativer Primärtherapie auf, so besteht noch die Möglichkeit einer Strahlentherapie mit kurativem Ansatz. Die 5-Jahres-Überlebensraten liegen hier bei etwa 25% [9, 10, 16, 21, 23, 33]. Andere Therapieansätze müssen für Patientinnen nach Operationen und adjuvanter Strahlentherapie oder für Patientinnen gesucht werden, die sich einer primären Strahlentherapie unterzogen haben. Gleiches gilt für alle die Patientinnen mit parametranen, bis zur Beckenwand reichenden Rezidiven, die entweder operativ nicht radikal entfernt werden können, oder bei denen durch die Vorbestrahlung aus Gründen der Strahlentoleranz der Organe im kleinen Becken wie Darm, Blase, Harnleiter, Nerven und Gefäße eine suffiziente perkutane und/oder intrakavitäre Bestrahlung nicht mehr durchführbar ist.
