Zusammenfassung
Bei 23 Psoriatikern wurde während einer zweiwöchigen Phototherapie (mit und ohne vorausgegangenen Solebädern) der Gehalt von Histidin (HIS) und der beiden Photoisomeren der Urocaninsäure (E-UA und Z-UA) im Stratum corneum an erscheinungsfreier und psoriatisch veränderter Haut gemessen und sowohl intraindividuell als auch mit einem Kollektiv Hautgesunder verglichen.
Im intraindividuellen Vergleich zeigte sich, daß psoriatische Haut hochsignifikant weniger E-UA (p<0,01) und Z-UA (p< 0,001) als unveränderte Haut von Psoriatikern enthielt; hingegen waren die Histidinwerte nicht signifikant verändert.
Der Vergleich von Meßwerten aus erscheinungsfreier Haut von Psoriasispatienten mit Werten, die früher an hautgesunden Kontrollpersonen (n = 30) ermittelt wurden, ergab keine Unterschiede bezüglich des Gehalts an Histidin, jedoch niedrigere Werte für E-UA und Z-UA bei Psoriatikern in unveränderter Haut und noch viel niedrigere in Psoriasiseffloreszenzen.
Der therapeutische Einsatz von Bädern mit gesättigter Sole (26%), wie sie in Bad Bentheim zur Anwendung kommen, führte zu einer Verminderung des Gehalts an Histidin (p<0,01), E-UA (p<0,05) und Z-UA (p<0,05).
Psoriatisch veränderte Haut ist offenbar nicht in der Lage, Histidin in relevanten Mengen in E-UA zu metabolisieren. Im Verlauf der Therapie steigt der Z-UA-Gehalt des Stratum corneum an, bedingt durch UV-induzierte Photoisomerisierung. Biochemische Analysen des Stratum corneum lassen sich mit der Klebefilmabrißmethode in unblutiger und nicht belastender Weise durchführen und bieten die Möglichkeit zur Objektivierung klinischer Therapieerfolge. Parallele Verläufe unter Photobalneo- und Phototherapie sprechen gegen die Hypothese von Dirnagl (7), daß die Eluierung von Urocaninsäure eine wesentliche Rolle im Wirkungsmechanismus der Photobalneotherapie spielt.
Summary
The amount of histidine (HIS) and of both photoisomers of the urocanic acid (UA) in stratum corneum on skin with and without psoriatic lesions was measured during a period of 2 weeks on 23 patients and compared both individually and also in a group of healthy-skinned individuals.
The intraindividual comparison showed that psoriatic skin has significautly less E-UA (p <0.01) and Z-UA (p < 0.001) than unlesioned skin of psoriatic patients; the HIS-counts were on the other hand not significantly changed.
The comparison of measures on unlesioned skin of psoriasis patients with amounts, which were formerly found on healthy control persons (n = 30), did not show any differences concerning the amount of HIS, although lower amounts for E-UA and Z-UA on psoriatic patients unchanged skin and much lower amounts in psoriatic lesions were found.
The therapeutic usage of baths with 26% salinic water, like is used in Bad Bentheim, caused a reduction of the amounts of HIS (p<0.0l), E-UA (p<0.05) and Z-UA (p<0.05).
Psoriatic lesioned skin is obviously not capable of metabolizing HIS in relevant amounts. During the therapy the amount of Z-UA of the stratum corneum rises due to UV-induced isomerisation.
Biochemical analysis of the stratum corneum can be done by using the stripping in a unbloody way and without discomfort to the patient and offers the possibility of guaranteeing the objectivity of clinical therapy successes.
Parallel courses under photobalneo- and phototherapy tells against the hypothesis of Dirnagl (7), that the eluation of UA has a great part in the effect mechanism of photobalneotherapy.
