Verhaltenstherapeutische Paartherapie bei Kinderwunschpatienten

Zusammenfassung

Ausgehend von Fragen zur Indikationsstellung und Effektivität von Psychotherapie bei Kinderwunschproblemen wird ein psychosomatisches Betrachtungsmodell menschlicher Fertilitätsstörungen skizziert. Demnach kann von einem negativen Einfluß psychosozialer Belastungen ausgegangen werden, der über psychobiologische Mechanismen auf die reproduktiven Systeme von Mann und Frau vermittelt wird, wobei partnerschaftlichem Streßverhalten eine wesentliche Mediatorenfunktion zuzumessen ist. Auf der Grundlage dieser Annahmen wird ein verhaltensmedizinisches Vorgehen vorgeschlagen, das über verhaltenstherapeutisch induzierte Veränderungen der partnerschaftlichen Interaktion Verbesserungen sowohl in der Fortpflanzungsfähigkeit als auch in der Bewältigung des unerfüllten Kinderwunsches durch das betroffene Paar verspricht. Entsprechende Hypothesen werden durch Ergebnisse einer experimentell kontrollierten klinischen Studie zur Effektivität von verhaltenstherapeutischer Paartherapie bei Kinderwunschpatienten unterstützt.

Summary

A psychosomatic concept of involuntary childlessness is outlined that focusses on questions of the indications for treatment and the effectiveness of psychotherapy for infertile couples. According to empirical research on the psychology of human sterility, psychosocial stress can be assumed to negatively influence human fertility via psychobiological effects mediated by behavioral patterns of coping and marital interaction. Results of a prospective clinical study on the effects of behavioral marital psychotherapy on infertility as well as on the coping abilities of sterile couples support hypotheses on the role of psychosomatic factors in fertility disturbances and indicate beneficial effects of behavioral marital therapy.
