Zusammenfassung

Die klinische Diagnose einer Rejektion nach orthotoper Leber-transplantation (LTX) stellt nach wie vor ohne die invasive Transplantatbiopsie eine schwierige differentialdiagnostische Erwägung dar. Eine Vielzahl kausal völlig differenter postoperativer Komplikationen wie Konservierungs- oder Ischämieschaden des Transplantates, Cyclosporintoxizität oder systemische als auch lokal-hepatische Infektionen können ein ähnliches klinisches Bild bedingen bzw. auch gleichartige pathologische Veränderungen im konventionellen Laborprofil bewirken. Die Interpretation der Transplantatbiopsie erweist sich darüberhinaus bei simultanem Vorliegen von unterschiedlichen Pathomechanismen oftmals als problematisch. Weitere sensitive und möglichst spezifische Labprparameter zur Differenzierung dieser Komplikationen wären von großem Wert. Die Serumanalyse von immunomodulatorisch auch bei diesen Zuständen wirksamen Cytokinen, deren löslicher Receptoren oder von T-Zell-Aktivierungsmarkern erscheint als ein plausibler theoretischer Losungsansatz (1, 2, 3, 4).
