Intraoperative Sonographie zum Nachweis okkulter Lebermetastasen beim kolorektalen Karzinom

Zusammenfassung

Etwa 15–35% der Patienten mit kolorektalen Karzinomen haben zum Zeitpunkt der Operation bereits Metastasen in der Leber [4, 5,15,16,21, 31]. Der Prozentsatz okkulter, d. h. intraoperativ nicht sichtbarer oder palpabler Lebermetastasen läßt sich nur schwer bestimmen. Finlay fand durch postoperative Computertomographie (CT) und Ultraschalluntersuchung in Verbindung mit einer genauen Verlaufsbeobachtung solche okkulten Lebermetastasen bei 24% der Patienten, die eine vermeintlich kurative Resektion ihres Kolontumors gehabt hatten [16]. Aufgrund von Fortschritten der resektiven bzw. chemotherapeutischen Behandlung von Lebermetastasen ist es sinnvoll, auch diese kleinen Herde möglichst frühzeitig und vollständig zu erfassen.
