Zusammenfassung

Der Beitrag behandelt psychische Probleme, die sich bei körperlich und psychisch gesunden Kindern aufgrund eines Krankenhausaufenthaltes ergeben können. Zunächst erfolgt eine Diskussion der diesem Forschungsbereich bislang zugrundeliegenden theoretischen Konzepte des „psychischen Hospitalismus“ und der „Bewältigung fremder Umwelten“. Unter dem Blickwinkel der Auseinandersetzung mit kritischen Lebensereignissen wird anschließend ein Rahmenmodell für die Verarbeitung von Krankenhausaufenthalten entwickelt. Auf der Seite der antezedenten Bedingungen werden dabei die Bereiche der Vorerfahrung des Kindes, der sozialen und psychischen Merkmale von Kind und Eltern und der bisher stark vernachlässigten kontextuellen Aspekte der Krankenhausstruktur (z. B. Rooming-in, klinischambulante Operationen) unterschieden. Die Auseinandersetzung und Bewältigung gliedert sich in die im Krankenhaus in einzelnen Belastungssituationen auftretenden Reaktionen und deren Auswirkung in der Zeit nach der Entlassung. Die Ergebnisse zu den einzelnen Bereichen werden kritisch kommentiert, so daß dadurch Einseitigkeiten und Lücken der bisherigen Forschung deutlich werden können.

Summary

This paper deals with the psychological distress caused by the hospitalization of physically and psychologically healthy children. “Psychological hospitalism” and “coping with strange environments” were discussed as basic theoretical concepts in this research area. Under the viewpoint of coping with critical life events, a model for coping with hospitalization has been developed. Children’s experiences with former hospitalizations, their own and their parents’ social and psychological characteristics, and hitherto neglected contextual aspects of hospital organization (e.g., rooming-in, out-patient operations) are mentioned as antecedent variables. Responses to specific situations during the stay in the hospital and their effects after discharge are indicated as parts of the coping process. A discussion of the outcomes in the different areas elucidates the shortcomings and gaps in previous research.
