Management of Fungal Infections in Patients with Neoplastic Diseases

Abstract

Systemic mycoses are life-threatening infections in granulocytopenic patients with neoplastic diseases (Horn et al. 1985; Walsh and Pizzo 1988). The problems of nosocomial outbreaks, clinical resistance, microbiological resistance, emergence of new pathogens, and difficulty in establishing an early diagnosis present formidable challenges to the successful management of these patients. Progress in treating these infections is being achieved through greater understanding of established available antifungal agents such as amphotericin B, flucytosine, miconazole, and ketoconazole. Promising investigational agents such as fluconazole and intraconazole will require careful clinical investigation in order to define their role in granulocytopenic patients. This chapter will review the management of fungal infections in granulocytopenic patients with neoplastic diseases.
