Zusammenfassung
Die Darstellung der hämodynamischen Auswirkungen supraventrikulärer Tachykardien und ihrer Behandlung bedarf der didaktischen Simplifizierung. Dies um so mehr, als zum einen unter dem Ausdruck supraventrikuläre Tachykardien unterschiedliche elektrophysiologisch begründete Rhythmusstörungen subsumiert werden — mehr als bei den ventrikulären Tachykardien — und sie sich zum anderen in der Vielzahl der Fälle als paroxysmale oder permanente Rhythmusstörungen bei sonst Gesunden manifestieren. Weiterhin werden die mechanisch-zeitlichen Beziehungen zwischen Vorhöfen und Kammern in unterschiedlicher Weise beeinflußt. Letztlich spielen periphere und auch kardiale Adaptationsmechanismen beim Nettoeffekt der hämodynamischen Veränderungen eine wesentliche Rolle. Ihre möglichen Interaktionen sind in Abb. 1 b aufgeführt. Danach zeigt die Tachykardie hämodynamische Auswirkungen über die Herzfrequenz selbst. Die Hämodynamik wird weiterhin bestimmt durch die Abstimmung von Vorhof-und Ventrikelsystole, während die ventrikuläre Erregungsausbreitung, die fast immer bei reinen supraventrikulären Tachykardien normal bleibt, in diesem Falle unberücksichtigt bleiben kann. Die elektromechanische Koordination wirkt auf und wird rückwirkend beeinflußt durch die Myokardfunktion, Koronardurchblutung und Ventilfunktion. Periphere und kardiale Adaptationsmechanismen bestimmen dann letztlich die Hämodynamik [38, 39].
