Zusammenfassung
Alle Strategien der Prävention und Behandlung einer Osteoporose werden bewertet nach ihrer Effektivität, die Fraktur zu verhindern. Da sich prospektive klinische Studien mit dem Parameter Frakturhäufigkeit in der Praxis kaum durchführen lassen, bevorzugen die meisten Autoren als Bezugspunkt die Knochenmasse. Viele epidemiologische Studien belegen den Zusammenhang zwischen Knochenmasseverlust und erhöhtem Frakturrisiko (Abb. 1). Mit zunehmendem Alter ändern sich auch die qualitative Knochenstruktur sowie das Belastungsprofil. Da unser Verständnis der Mechanismen des Knochenan- und -abbaus noch sehr lückenhaft ist, müssen alle kurzfristigen Beobachtungen von Veränderungen des Knochenmineralgehaltes und entsprechender laborchemischer Parameter mit Zurückhaltung interpretiert werden. Dennoch hat gerade hierdurch die Osteoporoseforschung einen erheblichen Fortschritt erfahren und erlaubt heute ein besseres Verständnis der Knochenphysiologie und ihrer therapeutischen Beeinflussung.
