Zusammenfassung
Bei Morbus Crohn wurde in den letzten 25–30 Jahren, v. a. in den hochindustrialisierten Ländern, eine starke Zunahme der Inzidenz beobachtet bei vergleichsweise unveränderter Inzidenz der Colitis ulcerosa [5, 14]. Diese epidemiologischen Beobachtungen richteten bei der Suche nach der Ätiopathogenese des Morbus Crohn das Augenmerk insbesondere auf Umweltfaktoren. Der Großteil diesbezüglicher Berichte (Assoziationen mit Nahrungsmitteln wie Milch oder Cornflakes; Übermaß oder Zuwenig an Ballaststoffen) ließ sich jedoch in der Folgezeit nicht bestätigen [9]. Auch die Diskussion über den Einfluß gehärteter Fette blieb bisher rein spekulativer Natur. Lediglich die wiederholt gemachten Beobachtungen einer gegenüber Gesunden signifikant kürzeren Stillperiode, einer signifikanten positiven Assoziation mit dem Rauchen sowie eines erhöhten Konsums raffinierter Kohlenhydrate bei Morbus Crohn hielten einer kritischen Überprüfung stand [9]. Diese Befunde sind in ihrer ätiopathogenetischen Wertigkeit jedoch unklar, zumal eine jüngere prospektive randomisierte Studie frühere Berichte über eine günstige Krankheitsbeeinflussung durch Verminderung des Anteils raffinierter Kohlenhydrate in der Nahrung nicht bestätigen konnte [12]. Wenngleich die Frage nach der Ätiologie der chronisch-entzündlichen Darmerkrankungen somit weiter unbeantwortet bleibt, trugen intensive Untersuchungen der Immunpathologie zu einem besseren Verständnis der Pathogenese der chronisch-entzündlichen Darmerkrankungen bei.
