Zusammenfassung
Die primär-kombinierte Strahlenbehandlung und Operation nach Wertheim/ Schauta stellen gleichwertige Behandlungsverfahren prinzipiell operabler Tumorstadien des Zervixkarzinoms dar. Grund für diese standardisierten, sehr radikalen Behandlungsverfahren ist die fehlende Information über die meist lymphogene Metastasierungspotenz des jeweiligen Tumors. Diese kann erst am histologischen Präparat (Lymphknotenbefall im Becken, Lymphbahn- und Gefäßeinbrüche, Primärtumorvolumen) abgeschätzt werden. Die unsichere prätherapeutische Stadieneinteilung nach der FIGO mit Fehlerraten zwischen 39 und 67% (Baltzer et al. 1980; Zander et al. 1981) sowie weitere diagnostische Stagingverfahren erlauben keine individuelle stadiengerechte Behandlung. So wird auch heute noch der größere Teil der Frauen mit Zervixkarzinom strahlentherapeutisch bzw. operativ übertherapiert, nämlich alle Frauen ohne pelvine Lymphknotenmetastasen, nach Liu und Meigs etwa 75% (Liu u. Meigs 1955) bzw. Baltzer etal. (1980) (Tabelle 1). Lediglich eine geringere Anzahl von Frauen wird wirklich adäquat radikal behandelt, und eine prognostisch besonders ungünstige Patientinnengruppe mit über das Becken hinausreichender lymphogener Metastasierung untertherapiert (Tabelle 2). Eine paraaortale Therapie, sei es nun Operation oder Strahlentherapie, stellt für diese Frauen eine erhebliche zusätzliche Belastung dar und ist verbunden mit einem deutlichen Anstieg an Nebenwirkungen (Piver u. Barlow 1977; Wharton et al. 1977; Potish et al. 1983).
