Zusammenfassung
In letzter Zeit wurde dem Krebsschmerz sowohl in der wissenschaftlichen Forschung als auch in der klinischen Praxis verstärkte Aufmerksamkeit zuteil. Dies betrifft jedoch fast ausschließlich die pharmakologische Schmerzbehandlung, während die psychologischen Probleme, die mit den vielfältigen Schmerzsyndromen, welche bei Krebserkrankungen auftreten können, weiterhin vernachlässigt werden. Gründe hierfür, die vorwiegend in Kommunikationsbarrieren sowohl auf der Seite des Patienten als auch des Arztes zu suchen sind, werden dargestellt und diskutiert. Psychische Probleme, die bei Patienten mit Schmerzen stärker und häufiger sind als bei Krebserkrankten ohne Schmerzen, sind als reaktive Störungsbilder zu verstehen. Entsprechend werden psychologische Schmerztherapien vorgestellt, die gleichzeitig Angst und Streß mildern: Schmerzimmunisierungstraining, Aufmerksamkeitslenkung, Entspannungsverfahren, Biofeedback, Musiktherapie und verschiedene hypnotherapeutische Verfahren.
Summary
Recently, cancer pain has become the object of increasing attention in both scientific research and clinical practice. However, this is true almost only for the pharmacological treatment of pain, whereas psychological problems resulting from a great variety of pain syndromes in cancer patients are still being neglected. The reasons for this situation, which is mainly due to a communication block in patients and in doctors, will be discussed. Psychological problems, which are more severe and more frequent in patients with pain than in cancer patients without pain, are to be seen as a reactive disturbance. Therefore, I want to present psychological pain therapies to treat fear and stress simultaneously: pain immunization training, attention control, relaxation, biofeedback, music therapy, and various hypnotherapeutic procedures.
