Zusammenfassung
N-Acetyl-β-D-glukosaminidase (NAG) ist ein lysosomales Enzym, mit einem molekularen Gewicht von 140000 Dalton, das in allen Zellen des Körpers, einschließlich der Tubuluszellen der Niere vorkommt. Da dieses Enzym wegen seiner Größe nicht filtriert wird, ist seine Anwesenheit im Urin auf die Sekretion aus den Tubuluszellen zurückzuführen. Eine Vermehrung der NAG-Sekretion in den Urin wird deshalb als diagnostisches Merkmal beginnender tubulärer Schädigungen diskutiert. So wurde eine Erhöhung der NAG-Aktivität im Urin als Frühsymptom der Nephrotoxizität von Medikamenten [2] bei diabetischen Nephropathien [6, 7] und bei Hypertonien [3] beschrieben.
