Zusammenfassung

„Das Argument des Dermatologen für eine systemische Therapie der Onychomykosen besteht gewöhnlich in dem Hinweis auf die unzureichende Wirksamkeit der rein lokalen Behandlung. Aber diese Argumentation widerlegt nicht die grundsätzliche Berechtigung der Forderung nach einer wirksamen Lokalbehandlung. Vielmehr verschärft es bei dem, der so argumentiert, das Gefühl, den Behandlungserfordernissen nicht gewachsen zu sein. So hat es nicht an Versuchen gefehlt, eine wirksame Lokaltherapie der Nagelmykosen zu entwickeln. Diese Bemühungen sind in den letzten Jahren verstärkt worden, und zwar einerseits im Zusammenhang mit der Entwicklung neuer Lokalantimykotika und zum anderen durch vermehrte Zuwendung zur Galenik des Harnstoffs als Keratolyticum“ [31].
