Über den Stellenwert der Gelenkszintigraphie in der Arthritisdiagnostik

Zusammenfassung

Bei ersten Untersuchungen über die Darstellung entzündlicher Gelenke mit Radionukliden wurden dieselben intraartikulär verabreicht. Hierbei fand man in arthritischen Gelenken einen beschleunigten Abfall der injizierten Radioaktivität (Ahlström et al. 1969). Diese Phänomen einer gesteigerten Nuklid-Clearance im Gelenk erklärt sich einerseits am ehesten durch einen vermehrten Blutfluß bei entzündlicher Hyperämie, andererseits durch eine gesteigerte Blut- und Lymphgängigkeit des Tracers. Diese Hypothese des vermehrten Abtransportes intraartikulärer Substanzen über den Lymphweg wird durch Befunde bei der Arthrographie von rheumatoid entzündlichen Gelenken gestützt, bei welcher wir immer wieder eine frühe Anfärbung der efferenten Lymphbahnen beobachten.
