Zusammenfassung
Viele Jahre der Forschung haben die Identifikation der Faktoren ermöglicht, die die Antigenität von Insulin bestimmen und kontrollieren. Aufgrund zahlreicher Studien gewannen wir die Überzeugung, daß freies Insulin und Insulinantikörper, wahrscheinlich durch Immunkomplexbildung ein Dysäquilibrium erreichen, das die Gewebsaffinität des Hormones reguliert. Ab einem gewissen Punkt unserer Untersuchungen wunderten wir uns und waren nahezu überzeugt, daß Immunkomplexe, mit oder ohne Insulin (Antigen), auf irgendeine Weise mit dem Auftreten früher diabetischer Sekundärkomplikationen, der „malignen Mikroangiopathie“ und einigen Komplikationen bei schwangeren Diabetikerinnen und ihren Kindern verbunden sein müssen. Später konnte gezeigt werden, daß die Immunogenität von Insulin nicht nur durch Speziesunterschiede, sondern auch durch Verunreinigungen selbst in kleinen Mengen verursacht sein kann und das Hormon stärker immunogen werden läßt. In der Folge erwies sich gereinigtes Schweineinsulin am wenigsten immunogen. Trotzdem wurden einige Fälle von Insulinallergie auch bei Verwendung dieser neuen Insuline berichtet. Einen weiteren Schritt auf dem Weg der Insulinreinigung und zur sicheren Anwendung von Insulinpräparaten stellt die Synthese von humanem Insulin durch gentechnologisches Rekombinationsverfahren mit Hilfe von Baktereien oder durch einen semi-synthetischen Prozeß mit chemischer Modifikation von Schweineinsulin dar. Aber auch unter Humaninsulintherapie wurden noch einige Fälle von Insulinallergie beobachtet. Vor wenigen Jahren wurde das spontane Auftreten von Insulinautoantikörpern bei Erstmanifestation eines Typ I-Diabetes mellitus, sogar den Verwandten dieser Patienten und auch anderen Personen mit einem erhöhten Risiko für Typ I- Diabetes beschrieben. In Analogie zu anderen Autoimmunkrankheiten könnte man den Schluß ziehen, daß das autologe Hormon die Bildung von Autoantikörpern hervorruft, die für die Pathogenese dieser Krankheit von Bedeutung sein mögen. Diesen neuen Antikörpern zusammen mit komplementfixierenden Inselzellantikörpern und Inselzelloberflächenantikörpern wird die Rolle von Markern zugeschrieben. Obwohl nicht absolut spezifisch, haben Anti-Insulinautoantikörper eine prädiktive Bedeutung für Personen mit erhöhtem Risiko für Typ I-Diabetes. Zuletzt soll noch erwähnt werden, daß in den vergangenen Monaten über die Fähigkeit von Anti-Insulinautoantikörpern, die Insulinsekretion zu stimulieren und eine Hypoglykämie auszulösen berichtet wurde.
