Zusammenfassung
Inseltransplantation als eine zuverlässige alternative Therapie für Typ I-Diabetes Mellitus bleibt eine vielversprechende Möglichkeit. Es gibt aber keine gut dokumentierten Fälle von erfolgreichen klinischen Versuchen. Trotzdem wurden entscheidende Fortschritte gemacht, um die Isolation der menschlichen Langerhansschen Inseln zu verbessern. Inselautotransplantation nach vollständiger oder nahezu vollständiger Pankreatektomie macht eine Langzeitfunktion transplantierter Inseln wahrscheinlich. Anfängliche klinische Versuche mit allogenen Inseltransplantaten haben eine kurzfristige endokrine Funktion aufgezeigt.
Verbesserte Isolationstechniken haben zu verbesserten Ergebnissen bei Inseltransplantationsversuchen geführt. Diese Versuche haben ebenso wichtige Aspekte für die zukünftige Forschung ergeben. Zur Immunologie der humanen Inseltransplantationen ist noch viel aufzuklären. Ebenso bleibt die Interaktion der immunosuppressiven Therapie mit dem Angehen der Inseln nach Implantation und ihrer Funktion weitgehend unbekannt. Weitere Forschung und Fortschritte auf diesen Gebieten könnten eines Tages ein sicheres System schaffen für eine kontinuierliche Insulinversorgung, mit der sich die physiologische Glukosehomöostase erreichen ließe. Vielleicht könnten mit einer solchen Therapie im Anfangsstadium der Erkrankung eingeleitet, die krankmachenden diabetischen Sekundärkomplikationen verhindert werden.
