Zusammenfassung

Die Behandlung der Herzinsuffizienz erfolgt mit Herzglykosiden oder neuen positiv inotropen Pharmaka sowie durch Einsatz von Vasodilatatoren. Trotz kritischer Äußerungen der letzten Jahre über den Einsatz von Digitalis bei koronarer Herzkrankheit, insbesondere bei Insuffizienz nach Myokardinfarkt, ist Digitalis nach wie vor zur Basistherapie der ischämischen Herzinsuffizienz indiziert. Eine endgültige Bewertung der neueren positiv-inotropen Substanzen ist noch nicht möglich. Die Behandlung mit Vasodilatatoren in Kombination mit Nitraten führt zu einer eindeutigen Reduktion der Mortalität der herzinsuffizienten Patienten.

Summary

The treatment of cardiac insufficiency is based on the use of glycosides or the new positive inotropic drugs, backed up with vasodilators. Despite recent criticism concerning the administration of digitalis in coronary heart disease, especially after myocardial infarction, digitalis glycosides are still indicated for the basic therapy of ischemic cardiac insufficiency. A final assessment of the newer positive inotropic substances is not yet possible. Treatment with vasodiltators combined with nitrates leads to a clear reduction of mortality in patients with cardiac insufficiency.
