Summary
Anorectal manometric studies were performed 42 times in 13 patients with rectal myectomy and 137 times in 49 cases treated by the PTCF method. The resting pressure of the anal canal was found to be lowered just after the operation and remained at levels lower than those in normal controls and in patients with rectal myectomy. This fact was attributed to the resection of the muscle layer in the anal canal. On the other hand, the resting pressure of the anal canal in patients undergoing the PTCF method decreased after the operation and recovered to normal levels within 3 years after the operation. This finding correlates with the clinical condition of bowel habits.
The resting pressure of the rectum elevated and remained high for more than 3 years in patients with rectal myectomy. This finding can be explained by the high tonus of the rectum oral to the aganglionic bowel which was left after rectal myectomy. The resting pressure of the rectum in patients treated by the PTCF method elevated just after the operation. It was lowered, however, to the normal level within about 3 years after the operation.
Regarding the data of basal rhythmic waves, no significant change was observed in comparison with normal controls.
The rectoanal reflex was observed only twice out of the 42 studies in patients with rectal myectomy and in nine of 137 studies in patients treated by the PTCF method. There was no relationship between the presence of reflex and clinical bowel habits.
Zusammenfassung
Anorektale manometrische Untersuchungen wurden 42mal bei 13 Patienten nach rektaler Myektomie und 137mal bei 49 Patienten nach „posterior triangular colonic Aap“ (PTCF) durchgeführt.
Der Ruhedruck im Analkanal war unmittelbar postoperativ erniedrigt und blieb unter den Werten gesunder Kontrollpatienten bei den Kindern mit rektaler Myektomie. Dies wurde auf die Resektion der Lamina muscularis im Analkanal zurückgeführt. Andererseits nahm der Ruhedruck bei Patienten mit PTCF nach der operation ab und kehrte binnen 3 Jahren auf Normalwerte zurück. Diese Befunde stimmen mit den klinischen Beobachtungen des Stuhlverhaltens überein.
Der Ruhedruck im Rektum war erhöht und blieb hoch über mehr als 3 Jahre bei Patienten mit rektaler Myektomie. Dieser Befund wird erklärt durch den hohen Tonus des Rektums proximal des aganglionären Darmes, der bei der rektalen Myektomie zurückblieb. Der Ruhedruck im Rektum bei Patienten, die mit PTCF behandelt wurden, stieg unmittelbar nach der Operation an, erreichte jedoch 3 Monate nach der Operation wieder Normalwerte.
Hinsichtlich der rhythmischen Wellenbewegungen waren keine signifikanten Unterschiede zu den gesunden Kontrollen zu beobachten.
Ein rektoanaler Reflex wurde nur 2mal biei den 42 Untersuchungen der Patienten mit rektaler Myektomie gefunden und nur 9mal bei den 137 Untersuchungen der Patienten, die mit der PTCF-Methode behandelt wurden. Es fand sich kein Zusammenhang zwischen der Anwesenheit des Reflexes und dem klinischen Stuchlverhalten.
Résumé
Des examens par manométrie rectale ont été effectués 42 fois chez 13 patients après amputation rectale et 137 fois chez 49 patients après transposition colique (PTCF, posterior triangular colonic flap).
La pression de repos dans le canal anal avait baissé immédiatement après l’opération et, dans le cas des enfants ayant subi une amputation rectale, elle resta en dessous des données obtenues chez des patients témoins. Cela fut imputé à la résection de la lamina muscularis dans le canal anal. D’autre part, la pression au repos chez les patients avec PTCF augmenta après l’opération et redevint normale en l’espace de 3 ans. Ces données confirment les observations cliniques de la fonction de défécation.
La pression au repos dans le rectum a augmenté et est restée trop élevée pendant plus de 3 ans chez les patients ayant subi une amputation rectale. Cela s’explique par la tonicité élevée du rectum au niveau de l’intestin aganglionnaire, préservé par l’amputation rectale. La pression de repos dans le rectum chez les patients traités par PTCF augmenta immédiatement en post-opérative mais retrouva un niveau normal 3 mois après l’intervention.
En ce qui concerne les ondulations rythmiques, aucune différence significative n’a été notée avec les témoins sains.
Un réflexe recto-anal n’a été observé que 2 fois sur 42 examens de patients ayant subi une amputation rectale et 9 fois seulement sur les 137 examnes de patients traités selon la méthode PTCF. 11 ne paraît y avoir aucun rapport entre la présence du réflexe et l’observation clinique de la défécation.
