Summary
A single-chamber pressure probe for rectal electromanometry was developed which seems to be superior to the complicated multichamber systems not only for clinical but also for experimental purposes. Measurements of rectoanal reflex were carried out in 268 cases with abnormal bowel function, in 103 cases following Operation for Hirschsprung’s disease, and in 61 cases of imperforate anus to assess postoperative continence. Experimental studies were performed in 36 dogs, 27 of which were used for short-term and 9 for long-term studies. The results of clinical and experimental studies are described and discussed, with accompanying literature. From clinical and experimental studies, the neuronal pathways of the rectoanal reflex are schematized. The normalrectoanal reflex is mediated by both the sacral cord and the myenteric neurons. It is concluded that measurements of the anal resting pressure and the rectoanal reflex constitute a valuable method to distinguish between normal and pathological sacral and myenteric innervation.
Zusammenfassung
Eine Einkammerdrucksonde zur Elektromanometrie wurde entwickelt, die nicht nur für klinische, sondern auch für experimentelle Untersuchungen Vorteile gegenüber den komplizierten Mehrkammersystemen aufzuweisen scheint. Messungen des rektoanalen Reflexes wurden bei 268 Fällen mit abnormer Darmfunktion, bei 103 Fällen nach Megacolon-congenitum-Hirschsprung- Operation und bei 61 Fällen von Analatresie zur Bestimmung der postoperativen Kontinenz durchgeführt. Experimentelle Untersuchungen wurden an 36 Hunden vorgenommen, und zwar für 27 Kurzzeit- und 9 Langzeitstudien. Die Ergebnisse der klinischen und experimentellen Messungen werden dargestellt und zusammen mit Angaben aus der Literatur diskutiert. Aufgrund von klinischen und experimentellen Studien wurden die neuronalen Verbindungen des rektoanalen Reflexes dargestellt. Ein normaler rektoanaler Reflex wird sowohl durch präsakrale als auch myenterische neuronale Elemente vermittelt. Die Bestimmung des analen Ruhedrucks und des rektoanalen Reflexes ist eine verläßliche Methode, zwischen normaler und pathologischer präsakraler und myenterischer Innervation zu unterscheiden.
Résumé
Une sonde manométrique à une seule chambre pour l’électromanométrie vient d’être mise au point. Ses avantages semblent certains, tant du point de vue expérimental que clinique, par comparaison avec la sonde à chambres multiples plus compliquée. Dans 268 cas de function intestinale anormale, 103 cas d’intervention chirurgicale pour maladie de Hirschsprung et 61 cas d’atrésie anale, des mesures du réflexe recto-anal pour déterminer le degre de continence post-opératoire ont été effectuées. Les etudes expérimentales ont été faites sur 36 chiens. Il y a eu 27 études de courte durée et 9 études de longue durée. Les résultats des mesures cliniques et expérimentales sont présentés et discutés en function de la littérature disponible. Le mode de transmission de l’influx neuronal traduisant le réflexe recto-anal a été déterminé par les études cliniques et expérimentales. Un réflex recto-anal normal est déclenché à la fois par la chorde sacrle et les neurons myentériques. La mesure de la pression anale repos et du réflexe recto-anal est une méthode fiable permettant la distinction entre innervation normale et innervation présacrale et myentérique pathologique.
