Zusammenfassung
Dihydrocodein wurde von Skita und Franck [22] 1911 synthetisiert. Es leitet sich formell vom Codein ab, das im Vergleich zu Morphin eine geringere Affinität zu Opiatrezeptoren hat. Die analgetische Wirkung von Codein ist daher schwächer als die von Morphin. Bezogen auf die analgetische Wirkung ist die antitussive Wirkung von Codein stärker als die von Morphin, weshalb Codein vornehmlich zur Hustendämpfung eingesetzt wird. Mit der konstitutionsbedingten Abnahme der analgetischen Wirksamkeit geht beim Codein eine Abschwächung der Nebenwirkungen einschließlich des Suchtpotentials einher [13]. Die Reduzierung einer Doppelbindung im Grundgerüst des Codeinmoleküls durch Einführung von zwei Wasserstoffatomen ergibt Dihydrocodein, dessen analgetische Wirksamkeit gegenüber Codein deutlich größer ist. Ob das auf einer höheren Affinität zu Opiatrezeptoren beruht, ist bisher noch nicht untersucht worden.
