Antidepressiva in der Behandlung chronischer Kopfschmerzpatienten

Zusammenfassung

Die Zusammenhänge von Schmerz und Depression sind aus mehreren Gründen von besonderem klinischen Interesse. In verschiedenen Studien wurde bei 36–66% depressiver Patienten Kopfschmerz gefunden, bei 17–65% Brust- Herz-, bei 7–17% Rückenund bei 10–12% Extremitätenschmerz (Poldinger 1980). Zusammengefaßt dominiert die Lokalisation im Bereich von Kopf und Rumpf. Eingebettet in andere Phänomene depressiver Erkrankungen kommt Schmerz bei verschiedenen Depressionsformen häufig vor: In einer Serie von 161 depressiven Patienten, die in eine psychiatrische Klinik in Schweden (von Knorring et al. 1983) eingewiesen wurden, litten 57% unter Schmerz, Frauen häufiger als Männer (64 vs. 48%). Bei einem Teil handelt es sich hier glaubhaft um schwere, unerträgliche Schmerzsyndrome, die diagnostisch nicht immer erkannt und entsprechend fehlbehandelt werden (Wörz u. Lendle 1980).
